Guidance for person-centered assessment and pathways in primary care

The second edition of the ICOPE guidance for person-centred assessment and pathways in primary care (ICOPE handbook, second edition) aims to support health workers in primary care put the recommendations outlined in the ICOPE Guidelines (https://iris.who.int/handle/10665/258981) into practice. Reflecting field experience, the ICOPE handbook second edition details the four steps of the ICOPE care pathway in primary care (basic assessment, in-depth assessment, developing a personalized care plan and implementing and monitoring).
